Form

To access a form, open the form group editor to which your form(s) belong. The forms in a group are listed at the bottom left of the questionnaire group editing screen.

A form is composed of sections, which themselves consist of questions.

Form Types

Each form has a type that indicates its usage within the questionnaire group:

  • Bound : associated with a questionnaire group
  • Unbound : not connected to a group
  • Unused : no active usage in the system

It is possible to filter the group's questionnaire list using the options at the top of the list.

Note : If a form you want to use appears as unbound or unused, this means it is not properly connected in the group's decision tree.

Add a Questionnaire

To add a questionnaire to a questionnaire group, select the "Add a questionnaire" section. Three options are available :

  • New questionnaire : create a blank questionnaire, then add your sections and questions according to your needs
  • Existing questionnaire : name your questionnaire and copy the structure of a questionnaire already present in the group
  • AI questionnaire : specify a medical specialty and, if desired, a consultation reason as well as additional information. You can also select up to three existing questionnaires to inspire the AI in generating the new questionnaire

Edit a Questionnaire

To rename or delete a questionnaire, simply right-click and choose the action you want to perform. To edit its content, a left-click opens the questionnaire.

Sections

A section is a set of questions. Each questionnaire must contain at least one section to be functional. By clicking on the pencil icon associated with a section, you can rename the section, modify its display order, or delete it using the trash icon.

Questions

For each section, you can edit existing questions by clicking on the pencil icon associated with the question or add new ones according to your needs.

Add a New Question

To add a question, you have two options :

  • Add a question : you create an original question adapted to your specific needs
  • Import a question : you choose an existing question from another questionnaire

The buttons for adding questions are located at the bottom of each section. Each new question is automatically placed at the end of the section where it was added, but you can later reorganize the order of questions according to your clinical logic.

Question Configuration

For each question, you can :

  • Enter the question to be answered
  • Choose the type of question :
    • Single checkbox → Only one answer checkbox
    • Multiple checkboxes → Multiple answer checkboxes possible
    • Mixed checkboxes → If the first checkbox is checked : single answer; otherwise : multiple answers
    • Text → Free text input for a textual answer
    • Date → Date selection
    • Time → Date and time selection
    • Photo → Photo upload
  • Define answer options if necessary
  • Assign a score to the question, with a specific weight for each answer
  • Add conditions so that the question only appears in certain cases, based on previous answers
