Questionnaires

What the patient sees

The questionnaire steps from the patient's side, from opening the link to submitting.

Knowing what the patient sees helps you guide them, over the phone or at reception. The questionnaire opens in their phone or computer browser, with no account or app to install.

The steps

Welcome

The Welcome to your pre-consultation questionnaire screen introduces the process. The patient can choose French or English and a light or dark display.

Identification

The patient provides the information your organization asks for: name, date of birth, contact details, health card number, etc. By continuing, they accept MedAssistant's terms of use and privacy policy.

Professional being consulted

If your clinic has several professionals, the patient chooses the one they are seeing, or I don't know. The report then arrives in that professional's inbox.

Reason for consultation

The patient indicates why they are consulting. This choice steers the questionnaire to the right questions.

Questions

The questions adapt to their answers: a follow-up question only appears when it is useful. Some ask for a photo (taken with the device or chosen from the gallery), others for reading a document to the last page. An important notice may appear before they start.

The Confirm sending the form screen presents your organization's consents. Depending on the questionnaire, the patient confirms their identity with a 6-digit code received by email or text message, then signs. The message Form successfully sent! confirms the submission.
