Questionnaires

Questionnaires overview

The patient tells their story before the visit; you read a structured report.

A pre-consultation questionnaire gathers the patient's history before they walk into your office. The patient fills it in at home or in the waiting room, at their own pace; the questions adapt to their answers. You receive a structured report in your inbox, with relevant negatives and a summary.

Why use them

  • Save time: the basic history is gathered before the consultation, with nothing left out, and you can focus on the assessment.
  • Independent patients: most patients fill in the questionnaire on their own, without tying up your staff.
  • Prepared patients: the patient thinks about their symptoms and questions before seeing you.
  • A more complete note: record the consultation with the Scribe from the report, and the note takes the patient's answers into account.

How it works

You send the questionnaire by email, text message, QR code or link, or have it filled in at the clinic on a tablet. See Send a questionnaire and Fill in at the clinic.

The patient answers: identification, reason for consultation, then questions tailored to their situation. See What the patient sees.

You read the report in the inbox, with its scores, flags and AI summary. See Read the report.

Which questionnaires

The questionnaires available depend on your organization's tools: family medicine, specialties, nursing, mental health and other professions. Several include validated clinical tools, such as the GAD-7, the PHQ-9 or the Epworth Sleepiness Scale, whose score is calculated automatically.

  • The manager adds the questionnaires of their choice, at no cost, in Organization › Forms / Scribes.
  • Professionals with the Form editor permission adapt them to their practice in the editor.
  • Need a specific questionnaire? Write to us at [email protected]: our team can design it for you.
