Shared group elements
Identification, decision tree, notices, complements, consents, recommendations and AI summaries.
Open a group from the editor's Forms tab. The group's sidebar lists, under Shared by the group, the elements shared by all its questionnaires, then the questionnaires themselves.
Identification
For each item, choose None, Optional or Required: first name, last name, date of birth, legal guardian, sex, pregnancy, consent, address, city, postal code, phone number, email, health card number and social insurance number.
- Legal guardian is asked when the patient is under 14; date of birth must then be required.
- Pregnancy is asked for a person of female or other sex aged 12 to 55; sex and date of birth must then be required.
Decision tree
The tree steers the patient to the right questionnaire. Each step asks a question (for example "What is the reason for your visit?"); each option leads to a questionnaire or to another step.
- Add a step, then its options, in French and English.
- Errors are flagged before saving: a tree with no starting step, a step with no option, an answer that leads nowhere, an unreachable step.
- Patient preview shows the tree as the patient goes through it.
- Statistics shows the number of answers per option, in your organization or across all of them.
- Each save creates a new version of the tree.
A questionnaire must be in the tree
A new questionnaire is only offered to patients once it is linked to the tree. The group's list shows this: Linked to the decision tree, Not linked to the decision tree or Unused. Filter the list with Linked, Not linked or Unused.
Limits: 100 steps and 19 options per step.
Notices
A notice is shown to the patient before they start: instructions, a warning, useful information. Write the title and the message in both languages, then associate the notice with the relevant questionnaires. A questionnaire has a single notice; a group, up to 20.
Complements
A complement adds shared questions before or after the questionnaire, for example medical history or lifestyle habits. A questionnaire can have several complements; a group, up to 20.
Consents
A consent presents, at the end of the questionnaire, boxes that the patient must check. Organize it into sections with a subtitle and items, in both languages. Authentication required also asks the patient to confirm their identity with a code received by email or text message; the identification must then ask for the email or the phone number.
A questionnaire has a single consent; a group, up to 20. To delete a notice or a consent, first remove its associations.
Recommendations
Recommendations are suggestions (test, referral, advice) associated with specific answers. They are shown to the professional in the report's Tools panel. Their order sets their priority. A group can have up to 100.
AI summaries
Instructions that guide the AI summary of each completed questionnaire in the group: style, format, items to highlight.
