Questions and questionnaires

Create a questionnaire, write its questions, add conditions and scores, and manage its versions.

A group's questionnaires are listed in the group's sidebar. Click a questionnaire to open it in the question editor.

Create a questionnaire

Choose New questionnaire, then:

  • Blank questionnaire, or a copy of a questionnaire in the group;
  • the AI questionnaire generator: enter the Reason for consultation (for example "Low back pain") and, if needed, Additional information such as the red flags to cover. The AI writes the questionnaire and derives its name from it.

The English name serves as the identifier: 30 characters at most. If it matches an existing questionnaire, creating it adds a new version of that questionnaire.

Don't forget to link the new questionnaire to the decision tree.

Write the questions

A questionnaire is made of sections, which contain the questions. The Questions, Notice, Complements and Consent tabs show its content.

  • Display language switches between French and English. The % translated indicator shows what remains to be translated.
  • Drag a question to move it, or use Alt + ↑ ↓. A question that depends on another must stay below it.
  • Import brings in questions from another questionnaire in the group, along with the questions they depend on.
  • The preview shows the questionnaire as the patient will see it.

Limits: 10 sections, 150 questions and 280 characters per text.

Question types

TypeAnswer
Single choiceOne answer only
Multiple choiceSeveral answers
Mixed choiceThe first answer excludes the others (for example "None of these symptoms"); otherwise, several answers
Free textUp to 280 characters
PhoneA phone number
Date, Date and timeA calendar
PhotosUp to 10 photos, optional or required

For each answer option, you can check Relevant negative (the unchecked option appears in red in the report) and associate a Recommendation with it.

Display conditions

Show the question: Always, If at least one condition or If every condition. A condition is based on the answer to a choice question placed higher up. This is how a follow-up question only appears when it is useful.

Scores

Count in a score adds up points based on the answers, with a weight per answer. On the score's last question, give a Final score title and interpretation ranges (for example 0 to 4 points: minimal). The editor flags gaps between the ranges. The total and its interpretation appear in the report.

Saving and versions

  • Save with the button or ⌘ S (Ctrl S on Windows). Errors to fix block saving.
  • Each save creates a new version. Answers already received keep the version they were filled in with.
  • The questionnaire menu shows the version, its date and the number of answers, and offers:
    • Rename;
    • Go back to version N: the latest version is permanently deleted;
    • Delete: all versions are deleted. This is not possible while the questionnaire is linked to the tree, if it is the last one in the group, or if it has unsaved changes.
